There are a few ways you can register.

Option 1 Download the Class Registration Form or the Camp Registration Form, fill out on your computer and e-mail to info@creativityunhinged.com.

Option 2 You may also mail a printed and signed copy of the Class Registration Form or the Camp Registration Form with check payment (payable to “Rumphius Creativity Unhinged”) to: 5A GOULD RD. WESTFORD, MA 01886.

Option 3 Send an e-mail to info@creativityunhinged.com with the following information:

• Class or Camp title, day and time that you wish to register for
• Name(s) of student(s) and birthdate(s)
• Name(s) and contact information of parent(s) or guardian(s), including address, all phone numbers, and e-mail addresses
• Emergency contact information
• Who is authorized to pick your child up from Summer Camp
• Allergies and other pertinent medical information Emily may need to know about the student(s)

Option 4 If you would like to register your child for classes or camp but have questions or concerns, give Emily a call at 978 995 7002. She would love to talk to you about Rumphius! You are welcome to register by phone.

Once registration information is received by phone, e-mail or snail mail, you will receive a confirmation e-mail from Emily. This will include payment verification or instructions. She will require signature from all parents or guardians on the first day of class when a student is dropped off, if you did not mail your form. Payment is due by the first day of class, as well.
